Cataract surgery
Cataract surgery was an elaborate affair just 3 decades back. There were month-long restrictions and results were, at best, acceptable. It underwent an overhaul with the advent of “Phacoemulsification”. Wound size reduced from 8-10 mm to sub-2 mm. Painful sutures were done away with and surgical time was reduced to just 7-8 mins. Soon the injection near the eye vanished and patients could see immediately after the surgery with no eye pad with the use of topical anesthesia. Not just cataract extraction, the intraocular Lens also evolved at the same pace. Soon there were lenses to correct cylindrical power- Toric IOLs & near & distance vision – Multifocal IOLs. Cataract surgery has become a Refractive Surgery with the promise of spectacle freedom. Trifocal IOLs and extended depth of focus IOLs (EDOF) are the current popular options for spectacle independence.
Refractive surgery
Refractive surgery or Laser vision correction started with the advent of Lasik (laser-in-situ Keratomilensis) with the creation of a corneal flap and corneal ablation below it with the Excimer laser. It evolved with better Microkeratome & laser machines. However, the real breakthrough was the introduction of the Femtosecond Laser. This was a cutting Laser that could fashion better, precise LASIK flaps and then went on to create Lenticules to do away with the need for Excimer Laser altogether. Femtosecond Laser-assisted Cataract surgery (FLACS) allows Laser-enabled wounds and cataract opening & division to make challenging cases easier and improve precision in cataract surgery.
